1. Please complete form at least 2 weeks prior to your event.
2. By completing this form, you are consenting to have your ministry charged for the items that you are requesting.
3. Please contact Douglas Wright at firstname.lastname@example.org for any additional needs or questions.
4. Cleaning & Sanitizing of all kitchen surfaces and equipment must be completed at the end of your event.
5. All leftover food must be removed from the kitchen at the end of your event.
6. Any group leaving the kitchen unclean will be contacted for further cleaning or charged a cleanup fee of $75.00
